The Foundation For Senior Care, Care Advocacy Program links client's and/or client's families with resources. As a Care Advocate Intern, you will develop case management skills and will have one on one meetings with client's and/or their family members. As a Care Advocate Intern you will develop assessment skills, build positive rapport with clients, set treatment goals, link client's and/or their family members with resources, inquire how the resources is working for the client, write progress notes and complete any other agency tasks. Interns must feel comfortable meeting with client's and/or families in the office or at the client's home. Interns will not conduct home visits alone. This internship focuses on disabled adults and the geriatric population.
